What to Bring to Your DOT Physical

Please review this checklist and bring any necessary documentation for conditions that apply to you to ensure a hassle-free DOT exam.

Dot Physical Exam

For all DOT exams, please bring a valid ID, a copy of your current medications, glasses and hearing aids if used to drive.

High blood pressure (Hypertension)

Medication list and treating provider information.

Diabetes

For Type 1 or insulin treated Diabetes please have your treating provider complete this form prior to your exam: MCSA-5870
For Type 2 or non-insulin treated diabetes please bring your last Hemoglobin A1C and medication list.

Sleep Apnea

Pleas bring a copy of your CPAP compliance report (minimum 30 days).

Heart Disease

If you have a history of heart disease, please bring a clearance letter from your cardiologist and most recent stress test. This includes conditions such as a heart attack, angina (chest pain), heart failure, irregular heartbeat (arrhythmia or atrial fibrillation), pacemaker or defibrillator placement, heart valve replacement or repair, coronary artery bypass surgery, angioplasty or stent placement, cardiomyopathy, or congenital heart defects that require ongoing monitoring.

Chronic vision issues

Please have your Opthalmologist complete form MCSA-5871 for conditions such as monocular vision (vision in only one eye), glaucoma, or any other vision issue that is being monitored.

Seizure disorder

Please apply for the seizure exemption through the FMCSA and bring a letter from your Neurologist.

Respiratory conditions (asthma, COPD)

Please bring clearance letter from Pulmonologist and pulmonary function test results.

Kidney disease

Please bring clearance letter from Nephrologist and recent lab results.

Mental health conditions

Please bring Medication list and clearance letter from treating provider.
